The following forms are MANDATORY for all First-Year student-athletes to complete prior to medical clearance for varsity sport participation at Bates College.

- Immunization Form
- HIPAA Form
- Emergency Information Form
- Concussion and Injury Agreement Form
ADD/ADHD Student-Athletes:
The following forms are MANDATORY for all First-Year varsity athletes that are diagnosed with ADD/ADHD and are on medication that is a NCAA banned substance
