Sample application for enrolled students.  Faculty adapt this application for the specific program.  Programs have an “early application date” at the end of the fall semester and a “regular application date” on the first Friday in February.

Student Forms

Agreement of Participation Form Each student submits this form to the Off-Campus Study Office upon acceptance to confirm their understanding of the program structure, behavior expectations, financial aspects of the program, and roles of the faculty directors.

Student Information and Health Form This form records personal and health information which is essential for the faculty directors and the College to have. Students complete this form, which requires passport and insurance information as well as parent notification, after being accepted to the program, and return it to the faculty leading the program.

Pre-Program Travel Waiver Occasionally, students want to travel on their own to the program. If faculty are going to allow this, they need to have the students complete the pre-program travel form.

Post-Program Travel Waiver Occasionally, students want to travel on their own after the completion of the program. If faculty are going to allow this, they need to have the students complete the post-program travel form. If such plans develop during the program, faculty should have the students complete the form and return it to them.
