Making Changes to your Elections

Changes through the Garnet Gateway Online Enrollment:

First sign into the Garnet Gateway and access online enrollment from the Employee Menu.

If all you want to do is change is your Healthcare or Dependent Care Reimbursement Account elections you can do that by starting your online enrollment through the Garnet Gateway.  Once you start your enrollment simply make your reimbursement account elections under the Healthcare and Dependent Care Reimbursement Account elections link.  Remember to submit a  completed Dependent Care Subsidy form to Human Resources if you are electing to contribute to the Dependent Care Account.

You may make changes to your medical plan by first going into your current election and stopping it.  Then make your new medical plan election.  If you cover a domestic partner make sure you elect options (5) You & Domestic Partner or option (6) You, Partner & Child(ren) as applicable.  If adding or deleting dependents make sure you elect the applicable level of coverage (1) You Only, (2) You & Spouse, (3) You & Child(ren), (4) You & Spouse & Child(ren), 5) You & Domestic Partner or option (6) You, Partner & Child(ren) and submit a completed Dependent Coverage Change Form to Human Resources.

To add or delete a dependent from your Dental Plan elect the applicable level of coverage (1) You Only, (2) You & Spouse, (3) You & Child(ren), (4) You & Spouse & Child(ren), 5) You & Domestic Partner or option (6) You, Partner & Child(ren) and submit a completed Dependent Coverage Change Form to Human Resources.

If you have coverage now but are opting out of medical or dental in 2012 you will need to submit proof to Human Resources of the other coverage – typically an ID card or enrollment form.

Life Insurance

If you want to change your life insurance elections you must do so by scheduling an appointment with Human Resources.  Please contact Ken Emerson at x6140 or Brenda Sawyer at x6176.

I have listed below what is involved in making changes to your life insurance.

Life Insurance:  During the annual enrollment period you have the opportunity to increase or decrease your life insurance.  If you increase your life insurance you will need to complete a Personal Health Statement.  Coverage would only be effective following UNUM’s approval.  UNUM may request more information depending on the results of their review.  Note, if you have basic life insurance and your salary is greater than $50,000 the increase in your basic life insurance due to the elimination of the $50,000 maximum does not require completion of a personal health statement.  The increase will automatically happen.

Accidental Death & Dismemberment (AD&D):  You can increase or decrease your AD&D coverage at Annual Enrollment without having to submit any health information.

Spouse/Partner Life and AD&D Insurance:  Your spouse/partner Life Insurance Coverage cannot exceed 100% of your own coverage.  If you have Life Insurance in the amount of $10,000 or $20,000 you can increase it to $30,000 without having to submit a Personal Health Statement.  If you are applying for coverage for your spouse or partner for the first time or for coverage in excess of $30,000 they will need to submit a personal health statement.

Your Spouse/partner AD&D coverage cannot exceed 100% of what you have on your self.  Similar to your own coverage you can increase or decrease your Spouse/Partner AD&D Coverage without submitting any health information.

Child(ren) Life and AD&D Coverage:  You can increase your Child(ren) life coverage to $15,000 at any Annual Enrollment Period without submitting a Personal Health Statement.  To increase to $20,000 in coverage a Personal Health Statement must be completed.  You must have some Life Insurance Coverage on yourself to cover your child(ren).

You can increase or decrease your Child(ren) AD&D Insurance at any time without submitting any health information.  You must have some AD&D coverage on yourself to elect coverage for your child(ren).
