Salary Reduction Agreement
If you wish to change the amount of money or the percentage you are currently contributing to your Group Supplemental Retirement Annuity(GSRA) with TIAA CREF you will need to complete the “Agreement for Salary Reduction” form.
Instructions for Form
- Print your name.
- Put the dollar amount or the percentage amount you would like taken from your check per pay period. A dollar amount would be a set amount, i.e. $50, $63 ect…per pay period and a percentage would be a % of your gross pay per pay period.
- Put the check date you would like the amount to be deducted.
- Date you are completing this form.
- Your signature.
Return the completed form to the Human Resources department.
Note
- 2012 Annual Contribution Limit is $17,000
- 2012 Catch up for participants over age 50 is $5,500
