Bobcat Express Medical Transportation Request Form

The Bobcat Express Medical Transportation Request Form is intended for doctor and medical appointments, and physical therapy appointments only. The Bobcat Express operates this service, during the academic year, Monday through Friday 8:00 a.m. – 6:00 p.m. Whenever possible, please make your appointments during these times. Transportation for appointments outside these scheduled times, may not be available.

There may be times during inclement weather, vehicle breakdowns and other unforeseen circumstances where shuttle services have to be amended, delayed or canceled.

This form should be filled out at least 48 hours prior to your medical appointemnt. Bobcat Express reservations are processed Monday through Friday, during regular college business hours. Reservations received outside these times will be processed the next business day. Requests submitted outside normal office hours, with little or no prior notice, will likely not be received or processed in time for your appointment so please plan ahead. For requests where 48 hour notice is not possible, call Campus Safety (207) 786-6254 to discuss transportation options.

Your reservation is not complete until you receive a Google Calendar event. Upon receipt, check the listed details for accuracy and direct any questions or concerns regarding the reservation to the Transportation Coordinator (207) 786-6337. If you have not received a reply by the next business day (either via calendar invite associated with the request, or an email informing otherwise), call 207-786-8300 with enough lead time to make your appointment.

This request form is ONLY intended for NON-EMERGENCYmedical transportation and will be provided on a first come, first served basis.
