Media Consent Form

  1. Print form — Media Consent Form 2014 (PDF)
  2. Sign form — Before your media can be made public (uploaded to YouTube and/or posted on the Bates website), individuals featured within that media must first sign the Media Consent Form. Bates staff and faculty who request media recording services are responsible for ensuring that individuals featured within recorded media have completed and signed a Media Consent Form.
  3. Upload form — Signed Media Consent Forms can be scanned and uploaded using the form below.
  4. Please submit the form below only ONCE for each event (you can attach multiple media releases below).

  • This will be used as the title on all official web channels, DVDs and other distributed media.
  • This is the date on which the event occurred.
  • Not required but used by Vimeo, YouTube and Soundcloud to assist in searching for particular content. Suggestions include #podcast #departmentname #eventseriesname
