{"id":78,"date":"2020-08-06T12:49:50","date_gmt":"2020-08-06T16:49:50","guid":{"rendered":"https:\/\/www.bates.edu\/finance-administration\/?p=78"},"modified":"2020-09-29T10:01:19","modified_gmt":"2020-09-29T14:01:19","slug":"bates-testing-and-consent-form","status":"publish","type":"post","link":"https:\/\/www.bates.edu\/finance-administration\/2020\/08\/06\/bates-testing-and-consent-form\/","title":{"rendered":"Bates testing and consent form"},"content":{"rendered":"\n<p>Dear Colleagues,<\/p>\n\n\n\n<p>As indicated in the\u00a0<a rel=\"noreferrer noopener\" href=\"https:\/\/www.bates.edu\/hr\/covid-19-testing-for-employees\/\" target=\"_blank\">FAQ\u00a0<\/a>about employee COVID testing,\u00a0<strong>all employees must sign a consent form prior to their first test<\/strong>.<\/p>\n\n\n\n<p><strong>(1) <\/strong>The consent form is now available on the\u00a0<a rel=\"noreferrer noopener\" href=\"https:\/\/www.bates.edu\/garnet-gateway\/\" target=\"_blank\">Garnet Gateway<\/a>\u00a0(see the &#8220;to-do&#8217;s&#8221; on the left). Please use your online credentials to sign this form. The consent form needs to be completed ahead of testing.<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li><em>Please tend to this right away<\/em>. Last minute consent forms will add complexity. Please complete the consent form regardless of timing and frequency you expect to test.<\/li><\/ul>\n\n\n\n<p><strong>(2)<\/strong> If you are a manager and you have staff on your team that need paper copies, translated copies, or other specific needs, please reach out to\u00a0<a rel=\"noreferrer noopener\" href=\"mailto:mbenoit@bates.edu\" target=\"_blank\">Melissa Benoit<\/a>\u00a0for information.<\/p>\n\n\n\n<p><strong>(3)<\/strong> Key Timetable:<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>August 6: Consent form available on\u00a0<a rel=\"noreferrer noopener\" href=\"https:\/\/www.bates.edu\/garnet-gateway\/\" target=\"_blank\">Garnet Gateway<\/a><\/li><li>August 18-20: Baseline employee testing. Sign-up information forthcoming.<\/li><li>Week of August 24th: Student baseline testing.
