Dear Colleagues,

As indicated in the FAQ about employee COVID testing, all employees must sign a consent form prior to their first test.

(1) The consent form is now available on the Garnet Gateway (see the “to-do’s” on the left). Please use your online credentials to sign this form. The consent form needs to be completed ahead of testing.

• Please tend to this right away. Last minute consent forms will add complexity. Please complete the consent form regardless of timing and frequency you expect to test.

(2) If you are a manager and you have staff on your team that need paper copies, translated copies, or other specific needs, please reach out to Melissa Benoit for information.

(3) Key Timetable:

• August 6: Consent form available on Garnet Gateway
• August 18-20: Baseline employee testing. Sign-up information forthcoming.
• Week of August 24th: Student baseline testing. No employee testing.
• August 31 forward: ongoing testing through the semester for students and employees.
