General FAQs about the Electronic Form

+How do I complete my health forms?

  • June 1st, 2017 FY Health Forms will be accessible through Garnet Gateway
  • There are two options:
    • Option 1: Complete them electronically through Garnet Gateway

      • Option 1 is the preferred method
    • Option 2: Print out the blank forms and manually fill them in


+What information will I need to complete the forms?

  • Your Personal Contact Information
  • Parent/Guardian Contact Information
  • Emergency Contact Information for at least one person
  • Primary Care Physician Contact Information
  • Bates Email Address
  • Insurance Card
  • Medical history
  • Medications you’re currently taking
  • Official Immunization Record from your physician’s office or from your high school


+Can I save my work so I can finish later?

  • Yes, when you click on “Finish Later” under the “Other Actions” menu item you will be able to come back to your document and finish it at a later date/time.
  • You should have received a security code, as well as a link to your document via email when you first verified your email address prior to filling out your form.
  • The link or “resume Signing” is what you need to edit your saved document.


+Can I print the completed electronic health form?

  • Yes, when you click on “Finish” to complete the form you will have an option to “Show” the document. You can then print it.
  • You will also receive a copy of the form in your email so you can view it and print it after completing the form


+What if I am under 18 years old?

  • You are required to have a Parent/Guardian fill out the Electronic Health Form

    • Under ‘Your Name’ when starting the electronic form the Parent/Guardian’s name must be used.
    • This is the name that will be used to sign the form
  • Under ‘Your Email’ the Student’s email address can be entered.


TB Assessment Form

+I answered 'yes' to one of the three questions in the section 'To Be Completed by Student'. Do I really have to get a physician to sign it?

  • Yes, you are required to have a physician complete and sign the bottom section of the form.


+Is it possible to print the TB form so I can send it to my PCP?

Yes, you can print the form using one of the following approaches:

  • Complete the electronic form without the physicians signature, then print just the TB form for sending to your PCP.
  • Complete the electronic form leaving the bottom section blank, then print the paper TB form to send to your PCP to be signed.

+Once my physician completes the bottom section, how do they send it to Bates Health Services?

  • Your physician can either Email (healthservices@bates.edu), mail or fax a copy by July 1 to:

  • Immunization Form

    +What if I don't know what the dates are for the first three Vaccine's, MMR 1, MMR 2 and the Tdap?

    • You will need to obtain a copy of your official Immunization Record from your physician or from your high school as ‘proof of immunization’ and send it to Bates Health Services after completing your electronic Immunization Form.


    +There is no place to enter text for the 'Include Proof of Immunization' section, what do I need to do?

    • Contact your physician and have them either Email (healthservices@bates.edu), mail or fax a copy of official Immunization Record by July 1 to:

    • Complete the electronic form leaving the bottom section blank, then print the Immunization form to send to your PCP to be signed.
