Annual Required Medical Plan Notice - view this notice for updated disclosure information on your Medical Plan and required annual notices.

Medicare Part D – Notices of Creditable Coverage - If you or a dependent are over age 65 and ending your coverage under the Bates Medical Plan you will want to print out the applicable Notice of Creditable Coverage for each year you were covered under the Bates Medical Plan after 65.  These notices will prevent you from having to pay a premium for Medicare Part D Coverage.

Notice of a Dependents COBRA Qualifying Event - Send this form to Human Resources to inform us that one or more of your dependents covered under the Bates Medical or Dental Plans is losing coverage.
