To request a payment for services, a completed IRS Form W-9 or W-8BEN must be submitted to Accounts Payable along with the Payment Request Form.

• International Individual Payee – Form W-8BEN through Adobe Sign
• International Company Payee – Form W-8BEN-E through Adobe Sign
